Download and complete one of our Test Request Forms and mail it with your payment to the address listed at the top of each form. Call 1-800-482-3025 to discuss pricing with our helpful representatives.
After receiving the request and the payment, Fairfax Identity Laboratories will notify the requester (unless otherwise indicated) by telephone, fax, or mail of the specimen collection appointment(s) for the individuals to be tested. Download Test Request From now!
